Purpose of the Assessment
This legally binding consent form is designed to obtain your informed consent for participation in a medical assessment conducted by Navi Consulting Group, operating under Navi Veritas Ventures Pty Ltd (ABN – 30 666 279 278). The assessment may include a comprehensive evaluation of your health through various methods, including telehealth consultations and the use of an AI medical scribe to document the consultation.

1. Nature of the Assessment
The medical assessment may involve a review of your medical history, physical and mental health evaluations, diagnostic tests, and other relevant medical procedures. The purpose of this assessment is to gather the necessary information to formulate a diagnosis, recommend treatment options, and manage your overall healthcare plan.
2. Telehealth and AI Scribe Usage:
The assessment may be conducted via telehealth, which allows you to consult with your healthcare provider remotely using secure electronic communication methods. During the telehealth consultation, an AI medical scribe may be used to assist in creating a detailed written record of the consultation. The AI scribe transcribes the conversation into text, which will be thoroughly reviewed and finalised by your healthcare provider before being added to your medical records.
3. Confidentiality and Privacy:
Data Protection: All information collected during the assessment will be kept confidential and secure in accordance with the Privacy Act 1988 (Cth) and the Australian Privacy Principles (APPs). Navi Consulting Group and Navi Veritas Ventures Pty Ltd are committed to protecting your personal and health information from unauthorised access, use, or disclosure.
AI Scribe Compliance: The use of the AI medical scribe adheres to the relevant privacy and security protocols required by Australian law. The AI scribe does not store audio recordings permanently; only the finalised text, after review by your healthcare provider, is retained in your medical record.
4. Your Rights:
Right to Withdraw: You have the right to withdraw from the assessment at any time without any negative consequences to your future care. Withdrawal of consent will not affect the quality of healthcare you receive.
Right to Review and Amend Records: You have the right to review and request corrections to your health records if you believe there are inaccuracies.
Right to Refuse AI Scribe Usage: You have the right to refuse the use of telehealth services or the AI medical scribe during your assessment. Alternative documentation methods will be employed if you choose to exercise this right.
5. Electronic Consent and Legal Validity:
Legal Framework: This consent form and the process of obtaining electronic consent are governed by the Electronic Transactions Act 1999 (Cth) and relevant state and territory legislation, ensuring that your electronic signature is legally binding and equivalent to a handwritten signature.
Consent for Data Use: By agreeing to this consent form, you authorize the use of telehealth services and the AI medical scribe, and you consent to the sharing of your health information within the healthcare provider’s network as necessary for your treatment. This consent also includes authorisation for the storage and processing of your health information in secure systems operated by Navi Consulting Group and Navi Veritas Ventures Pty Ltd, in compliance with the Privacy Act 1988 (Cth) and the Health Records Act 2001 (Vic) or equivalent legislation in your state or territory.
